The Indonesian Health Insurance Agency (BPJS Kesehatan) received Rp 60.58 billion in contributions from participants during the January-September 2018 period. However, the cost of healthcare benefits for participants reached Rp 68.53 trillion. As a result, the health social security fund experienced a deficit of Rp 7.95 trillion this year. This figure excludes operational costs.
Based on participant segments, informal workers contributed the largest deficit, amounting to Rp 13.83 trillion. This resulted from contributions totaling only Rp 6.51 trillion, while the cost of healthcare benefits reached Rp 20.34 trillion. The second largest deficit contributor was the non-worker segment, reaching Rp 4.39 trillion, followed by participants registered by local governments at Rp 1.45 trillion.

Meanwhile, participants from the formal private sector worker segment recorded the largest surplus. With contributions totaling Rp 18.08 trillion, healthcare benefit payments amounted to only Rp 10.27 trillion, resulting in a surplus of Rp 7.8 trillion. Furthermore, participants categorized as poor and impoverished experienced a surplus of Rp 3.21 trillion, with contribution income of Rp 19.1 trillion and healthcare benefit costs of Rp 15.9 trillion. Participants from the Civil Servant (ASN), Police, and TNI segment recorded a surplus of Rp 706 billion, with contribution income of Rp 10.63 trillion and costs of Rp 9.92 trillion.
