Number of BPJS Health Participants for the 2019-2025 Period

The number of participants in the National Health Insurance (JKN) program managed by BPJS Kesehatan continues to increase.

In 2019, the total number of participants registered in this program was just 224.1 million people.

Subsequently, the trend fluctuated with an upward trend, reaching 282.7 million people in 2025, as shown in the graph.

Despite the increasing total number, the proportion of active BPJS Kesehatan participants decreased, while the number of inactive participants increased.

In 2019, the proportion of active BPJS Kesehatan participants who paid premiums reached 91% of the total registered participants.

Only 9% of participants were inactive in 2019, either due to arrears in premium payments or because they had changed their membership category (mutation) and therefore stopped paying.

However, by 2025, the proportion of active participants had dropped to 83%, while the number of inactive participants reached 17% of the total registered participants.

According to Health Minister Budi Gunadi Sadikin, the increase in inactive participants was influenced by the discussion about cancelling BPJS Kesehatan premium arrears.

"This is the same as in banking. When there's a discussion about cancelling [arrears], people who were previously active end up becoming inactive because they hope it will be cancelled," said Budi at a meeting with Commission IX of the House of Representatives on Wednesday (11/2/2026).

Budi stated that the value of premium arrears of inactive BPJS Kesehatan participants until 2024 reached Rp26.47 trillion.

The largest arrears came from participants in the category of non-wage earners (PBPU) and non-workers (BP) who registered independently, with a total arrears value of Rp22.29 trillion.

Meanwhile, the smallest arrears until 2024 came from the group of participants receiving premium assistance (PBI), amounting to Rp84 billion.
